Do I need a referral to make an appointment at Revitalize Exercise Physiology + Pilates?

No you don’t require a referral. However, if your payments are to be paid by a 3rd party (ie. Medicare, WorkCover, TAC, DVA or NDIS), you, your coordinator or specialist will be required to complete the corresponding referral form.

Can I claim a private health rebate for attending Revitalize Exercise Physiology +Pilates?

Yes. Some private health funds recognise Exercise Physiology and Pilates services. Click HERE for the full list. We recommend that you check with your health fund as the rebate will depend on your level of cover.

Do you offer gym memberships?

Revitalize Exercise Physiology + Pilates is not a gym. We currently offer pilates & mixed group memberships for 1, 2 or 3 sessions per week.

Do I have to do anything before my session at Revitalize Exercise Physiology + Pilates?

Yes. Please complete the ‘Revitalize Intake Form’ which will be sent to you via email with a security code to protect your information. The form will take you 5-7 min and you will need your medications list, Medicare card number, DVA or private health insurance policy number and payment details. If you are seeing us through NDIS, Workcover/Worksafe & TAC please fill in all your relevant fields.

I have been referred by my doctor. What should I bring along to my initial assessment?

Please bring any X-rays, scans and tests relevant to your visit. Furthermore, any referral letters or forms. You will also need to wear comfortable clothing, walking shoes and a drink bottle.

Do you have any exercise equipment at your studios that I could utilise to help achieve my goals?

Yes. Though we are not a gym, we are fully equipped to support your rehabilitation process and have a fully equipped Pilates studio.

Are all of your staff accredited professionals with relevant credentials and insurances?

Yes, we have Accredited Allied Health Specialists and Advanced Pilates Instructors. All of our Exercise Physiologists meet the strict guidelines for full membership, accreditation and insurance with all relevant professional associations in Australia. 

Also, our Advanced Pilates instructors hold our highest accreditation and work alongside our exercise physiologists to assist and support you through your personalised program.

Furthermore, the team at Revitalize Exercise Physiology + Pilates are fully accredited with Medicare Australia, WorkCover VIC, Department of Veteran Affairs, NDIS, TAC and All Private Health Funds.

Is there any public transport available nearby?

Yes there is public transport available nearby.

  • Craigieburn Train – Stop at Essendon Station and a 2min walk
  • Buss 477 – Stop Raleigh St Essendon then 1 min walk

There are also plenty of uber and taxis around to help accommodate your
transport needs.

Where do I park when attending your studios?

  • Parking in the Mt.Alexander Rd slip lane up to 2hrs 
  • Parking on Napier St from 1-2hrs.

  • Off street parking on Duffy St, Essendon North.

 

For premises 15/180 Riverdale Boulevard, Mernda, 3754

  • Parking in the shop car park out the front of premises.

For premises 107 Hazel Glenn Drive, Doreen, 3754.

  •  Parking in the shop car park out the front of premises or 250 meters onwards.

Is there Wheelchair access?

Do you have credit card facilities?

Yes, we offer direct debit and eftpos payment options. Rebates are done electronically on the spot for Medicare and will clear in 24hrs into your bank account.

If I attend regular sessions how do I pay for my session?

If you are attending on a regular basis all sessions are charged on a Monday morning for the week ahead. Changes can be made to your account in advance or for the following week via sms or email. Please ask our friendly team if you have any further questions about our payment system.

What happens if I miss my appointment /session?

We will do our best to reschedule your session according to the terms of our cancellation policy.
